Lecture 1

Importance of the implant abutment connection for long-term stability and clinical success

The prosthetic treatment plan must allow an ideal integration of esthetics, biologics and functional requirements on natural teeth and on implants. In case of implants the prosthetic treatment is usually more challenging due to the inevitable soft tissues changing after teeth extraction. In fact, the soft-tissue healing and management around implants represents a crucial aspect and must be considered also from a prosthetic point of view in order to conclude the therapy with excellent outcomes. In this sense the prosthetic components assume an essential role in order to idealize the prosthetic integration and simultaneously achieve reliable and stable prosthetic results. Clinical situations require specific evaluations in terms of implant connection, prosthetic components and restorative materials and approaches. Nowadays, it is well known that the biological integration and stability around implants, can be significantly influenced by the correct use of modern prosthetic components. In this sense the use of not-removable prosthetic components allows to idealize the soft tissues healing and management around implant with significant advantage from a biological, biomechanical and clinical point of views. The On1 concept offers an undisturbed mucosal seal with many advantages for both surgeons and restorative clinicians. It provides surgical flexibility along with soft tissue biocompatibility, but also provides a simplified restorative workflow.

 

Lecture 2

Complete prosthetic protocol in edentulous patients

The edentulous predicament is directly related to alteration of facial esthetics and decrease in the lower facial height, as well as loss of ability to chew, taste, and smile. The rehabilitation of an edentulous maxilla with implants loaded immediately after placement with a full-arch prosthesis represents a valid treatment option that has shown a high rate of survival. Today, many implant surgical and prosthodontic concepts are used for the treatment of the edentulous jaws. The All on 4 concept, that is a modern strategic approach which employs tilted implants to restore edentulous patients, has been proposed as an alternative to bone augmentation procedures. The placement of four implants two posteriorly and two anteriorly makes it possible to avoid bone augmentation procedures when rehabilitating a completely edentulous arch with minimal bone volume. Independently from the surgical approach the treatment of the edentulous arches requires always specific clinical considerations in term of esthetics, occlusion, phonetics, function and biomechanics. All these evaluations will drive the clinician in the surgical procedures and in all the prosthetic and technical procedures necessary to finalize and maintain the case with a reliable prognosis. Nowadays the digital workflow represents a real breakthrough that allows us to work with an ideal prosthetic driven treatment, combining and evaluating simultaneously prosthetic and surgical aspects.
